Please complete this questionnaire questionnaire if you plan to enroll your child in the CCSD Pre-K Class - September 2026.

Your child must be 4 years old on or before December 1, 2026, and reside within the Canaseraga Central School District.

Pre-K Screening Day will take place in Spring 2026.

Information will be distributed to parents/guardians that complete this survey.

How to register your child for PreK in September:

1. Complete the PreK Screening Questionnaire, which is available each year starting in February and ending mid April.
2. The Student Services Office will use information compiled from the questionnaire to mail home a Registration Packet and schedule your child’s screening.

• If you are registering your child after PreK Screening has taken place, please contact the Student Services Office (ext. 2314) for an Enrollment Packet and be prepared to provide all information listed below.

Who is PreK Screening for?

• Children that are/will be 4 years old on/before December 1^st, 2026; and reside within the Canaseraga Central School District.

When is PreK Screening?

• The date varies each year, but it is usually towards the end of April.

What should I bring to PreK Screening? (Copies can be made at CCSD, if needed)

• Completed Registration Forms (will be mailed to you after completing the questionnaire)
• Child’s Birth Certificate
• Proof of Residency
• Health & Immunization Records – can be faxed directly to the Nurse 607-545-6073 before screening day
  • Please remember that Physical Exams must be less than 1 year old from the date of registration.
• Current Custody Agreement (if applicable)
